Integrated Community Staff Nurse - 0-19 Service Tower Hamlets

We have a wonderful and exciting opportunity in our 0-5 Universal Health team. We have 2 Fixed Term (Full Time) Community Nursery Nurse vacancy in Tower Hamlets. We are looking for people to join our progressive, innovative 0-5 service, and enhance their career by developing knowledge and skills about children and young people and families within Community settings.

Perinatal Community Nursery Nurse - Start 4 Life

Tower Hamlets is one of 5 pilots across the country delivering a workforce pilot to families in first 1001 critical days. We have 4 pathways and the pilot has been extended until 31st December 2025. We are looking for an experienced Community Nursery Nurse join the team and work with the perinatal lead in supporting families where the parents are experiencing mild to moderate mental health.

Health Visitor - 0-19 Service Tower Hamlets

Be responsible for providing the Healthy Child Programme and service to a defined caseload of families and to have a key role in assessing needs of clients, and planning and implementing appropriate interventions for families on the caseload, working collaboratively and referring, if necessary, to other health, Childrens Social Care and community agencies.

PCN Operations Manager 

We have a new exciting role for a PCN Operations Manager within the Primary Care Network, focused on supporting and improving the way the network operates.Tower Hamlets GP Care Group is a federation of 30+ General Practices which cares for more than 312,000 people. Network One is a Primary Care Network (PCN) of four GP practices around Bethnal Green in Tower Hamlets, East London, that serves around 50,000 patients in the borough.

Physical Activity Clinical Champion

We are seeking a Physical Activity Clinical Champion (PACC) to join an exciting Sport England-funded initiative piloting a place-based approach to the national PACC training programme. This role will focus on organising and delivering clinician-to-clinician training, empowering healthcare professionals to promote physical activity as a key intervention for the prevention and management of long-term conditions.
