Practice Development Nurse
The post holder will provide direction for the future of nursing through practice development and education, reflecting the strategic changes in the nursing profession. Through a process of work based competencies, the post holder will provide continuing support to all grades of staff in their delivery of nursing care, collaborating with multi-disciplinary teams and medical colleagues. The post-holder will provide effective, visible leadership, this includes leading locality-based teams and will therefore be expected to be work at sites across the borough.

Head Of Service - Tower Hamlets 0-19 Children’s Services
The Head of Service will play a key role in providing strong and inspirational leadership to the Children and Young People Services as part of an integrated approach across the Borough working in partnership with key stakeholders across the Children and Young peoples spectrum. The post holder will be expected to support their team, department, and organisation to achieve the organisations values in their day-to-day work.

Practice Based Clinical Pharmacist
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The education coordinator will support the delivery of the Community Education Provider Network (CEPN) programme through events coordination support including the planning, advertising and evaluation of the programmes. Healthcare Assistant - Physical Health
As a member of the GPCG Extended Access Hub team, your primary aim will be to provide physical health checks to a defined group of patients at their workplaces in Tower Hamlets. The workplace Health Check will encompass assessments of smoking status, height/weight/BMI, alcohol risk status, blood pressure check, and type 2 diabetes risk score.

The Specialist Practitioner will work with other services within Health, Social Care, Primary Care and across the Family Hub partnership to develop /strengthen evidence based perinatal pathways, training, policies, and process to clearly provide direction in relation to assessment and evaluation of care and ongoing support at the earliest opportunity.
