Job Vacancy: Social Prescribers (entry level) x 2 posts

Are you passionate about empowering patients? In the last 12 months, our Primary Care-led Social Prescribing service has successfully supported more than 4,000 patients to take better control of their own health and wellbeing and we are now recruiting for an additional two entry-level Social Prescribers to help take the service to the next level.

The Care Group has been delivering the hugely successful Social Prescribing service for more than three years, referring patients into 200 onward services. The service is now recruiting one full-time and one part-time Social Prescriber to join the team. The increased capacity will ensure our Social Prescribing service can expand to include our 0-19 services, Community Health Services, Urgent Care and Secondary Care Cancer Services.

 

What will the role involve?

The successful candidates will be responsible for:

  • providing one to one support
  • identifying issues that impact patient's health and wellbeing
  • developing personalised care and support plans to address people's needs
  • making referrals to community and voluntary sector services
  • recording patient outcomes

 

What are we looking for?

Candidates must have excellent interpersonal and communication skills, case management experience, care assessment planning and support skills. In addition, if you have knowledge of community voluntary & independent sector support services, and the ability to develop trust relationships to support people with complex needs, then we want to hear from you.
