Self-referral to Tower Hamlets FNP

Referrals are to be made as early as possible in the pregnancy, before the 28th week of the pregnancy.

You can refer yourself to the Tower Hamlets FNP programme by either:

• Filling in the self-referral form 
• Speaking to your midwife or GP 
• Calling the Tower Hamlets FNP team on 020 8223 8601.
