Referral Acknowledgement
You will receive an automatic response acknowledging that the referral has been received with a reference number and a message that the referral will be responded to within 5 working days as an initial response/contact by the A School Health and Wellbeing service.
Triage process
A School Health and Wellbeing service member of staff (school nurse or team leader) will undertake a review of the information to triage the referral to assess the urgency of the request and make contact with the referrer to discuss the requirements, agree on a date to meet the referring school staff member and parents/child to undertake an assessment/review of the child health needs/care plan.
