We have an exciting new role for an Assistant Director Integrated Pathway Development in the Care Group to develop and oversee the work associated with developing integrated pathways for services that may move from acute to community. You will work closely with health and care system partners driving change during this accelerated phase of transformation in line with NHS 10 Year Plan and Neighbourhood development.
